Approximately 100 pharmacies are expected to begin offering the service during autumn 2026, with the pilot running until spring 2027. This gives patients another way to assess a risk that is otherwise easy to overlook. So, what will the new NHS service do, and how does it work? ## **Who will be able to use the pharmacy service?** The pilot is intended for people aged 40 to 84 who are not pregnant and have not had a blood test during the previous 12 months. It will operate as a walk-in service, although only pharmacies taking part in the pilot will currently be able to provide it. The [pharmacist](https://www.suresign.com/ciga-healthcare-blog/dangers-of-heatstroke) places a small sample of blood from the fingertip onto a test strip or into a disposable cassette inside a point-of-care lipid analyser. Reagents within the test react with different fats in the blood, and the analyser measures those reactions to determine their concentrations. This allows the [seven-minute test](https://www.england.nhs.uk/2026/08/high-street-pharmacies-to-launch-seven-minute-cholesterol-checks/) to produce several related results from one sample. Depending on the analyser, total cholesterol, HDL cholesterol and triglycerides may be measured directly. The device can then use those measurements to calculate other parts of the patient’s profile. Non-HDL cholesterol is found by subtracting HDL from total cholesterol, while the total-to-HDL ratio compares the two figures. LDL and triglycerides may be measured directly or estimated from the other results. The appointment will also include a [blood pressure check](https://www.suresign.com/ciga-healthcare-blog/good-blood-pressure-readings), because cholesterol cannot be interpreted in isolation from other cardiovascular risk factors. A pharmacist prescriber will then review the results, offer the customer relevant lifestyle advice and decide whether treatment or further investigation is required. The customer will then receive a copy of the results by email. The service therefore combines the convenience of rapid testing with the clinical judgement needed to decide what should happen next. ## **Pharmacist review turns high street screening into community-level cardiovascular prevention** Cholesterol is a fatty substance that the body needs, but excessive levels can contribute to deposits inside the blood vessels and increase the risk of heart disease or stroke. Age, smoking status, family history and existing health conditions can help establish a patient’s general cardiovascular risk profile. However, because high cholesterol usually causes no symptoms, the pharmacy check may uncover a cardiovascular risk that neither the customer nor the pharmacy team could identify from outward signs. The results of an earlier north London pilot between January 2025 and March 2026 illustrate the clinical contribution pharmacies can make. Of the 2,493 people tested across 61 community pharmacies, 19% were considered suitable for pharmacological intervention and 88 started taking statins.
